Description

Danavir 600mg contains Darunavir, an antiretroviral drug used in the treatment of HIV-1 infection and AIDS.

  • Due to the risk of resistance, Darunavir is always administered in combination with other antiretroviral drugs such as Ritonavir or Cobicistat.

  • Darunavir is a protease inhibitor, blocking the HIV protease enzyme, preventing the formation of mature, infectious viral particles.

Medical Uses / Indications

  • HIV-1 infection in adults (in combination with Ritonavir, Cobicistat, or other antiretroviral drugs).

  • Pediatric HIV-1 infection (Darunavir + Ritonavir combination).

  • Therapy decisions are guided by plasma HIV-1 RNA levels and CD4+ counts.

Mechanism of Action

  • Darunavir inhibits HIV-1 protease, an enzyme essential for viral replication.

  • Prevents dimerization and catalytic activity of the protease.

  • Blocks cleavage of viral Gag-Pol polyproteins, preventing the production of mature infectious virus.

  • Its molecular flexibility allows it to remain effective against protease mutations.

Side Effects

Common:

  • Nausea, mild stomach pain

  • Insomnia

  • Itching, rashes

  • Fat redistribution (body shape changes)

Serious / Rare:

  • Allergic reactions (hives, swelling of face/lips/throat/tongue, difficulty breathing)

  • Severe hepatomegaly with steatosis

  • Chest discomfort, wheezing

  • Increased heart rate, lumbar pain

  • Cold sores

Precautions

  • Sulfonamide allergy: Use cautiously; Darunavir contains a sulfonamide moiety.

  • Drug interactions: May cause life-threatening reactions; avoid contraindicated combinations.

  • Hemophilia: Can increase bleeding risk (skin hematomas, hemarthrosis).

  • Diabetes mellitus: Monitor blood glucose; may increase glucose levels.

  • Fat redistribution: Risk of obesity; common in antiretroviral therapy.

  • Immune reconstitution syndrome: May occur; discontinue therapy if severe.

Contraindications / Drug Interactions

  • CYP3A interactions: Combination with Ritonavir increases plasma levels; monitor closely.

  • Other contraindicated drugs:

    • Alfuzosin → hypotension

    • Dihydroergotamine → peripheral vasospasm/ischemia

    • Cisapride & Pimozide → cardiac arrhythmias

    • Midazolam & Triazolam → respiratory depression

Pregnancy & Lactation

  • Pregnancy Category C: Animal studies show potential fetal risk; use only if benefits outweigh risks.

  • Breastfeeding: Likely unsafe; consult a doctor before use.

Dosage

  • Standard adult dose: 600mg orally twice daily (2 × 300mg) with food.

  • Once daily dosing for resistant HIV: 800mg Darunavir + Ritonavir once daily with food.

  • With at least one resistance-associated substitution: 600mg Darunavir + 100mg Ritonavir twice daily with food.

Administration: Oral, with food.

Storage

  • Store at room temperature (25°C).

  • Keep in an airtight container, away from children and pets.

Missed Dose

  • Take the missed dose as soon as possible before the next scheduled dose.

  • If near the next dose, skip the missed dose and continue regular schedule.
